To prepare for your visit, be ready to discuss your medical history and share any past x-rays or other useful information. Please bring your new patient form linked above to expedite check-in.
Dr. Mourad will review any previous medical records and will meet with you one-on-one to talk through concerns and goals and create a treatment plan tailored specifically to your needs.
Please arrive approximately 15 minutes prior to your consultation so that we can verify any information. A parent or guardian must accompany patients under the age of 18.
We accept most insurances. If you have questions about insurance coverage, or would like to talk through treatment plan options, someone on the 4D team will be able to assist you before or after your visit.
